Plateaus on a GLP-1: what the trial curves show, and what a stall usually is

You'll get: Weight loss on these drugs slows by design. The trial curves are steep early and flatten by about a year.

Weight loss on semaglutide and tirzepatide slows by design. Here is when the published curves flatten, why four flat weeks is not a plateau, what to check, and when to involve your prescriber.

Maintenance and stopping: what STEP 4, SURMOUNT-4 and the STEP 1 extension measured

You'll get: Three trials followed people after the drug stopped. On average, half to two-thirds of the lost weight came back within a year.

The three trials that followed people after semaglutide or tirzepatide was withdrawn, with the numbers, what they do and do not tell you, and how to turn them into a conversation with your prescriber.

Nutrition

Protein targets on a GLP-1: the ranges, where they come from, and how to hit them on a small appetite

You'll get: The 2025 advisory sets 1.2 to 1.6 grams of protein per kilogram a day, or 80 to 120 grams, while losing weight.

The 2025 joint advisory range of 1.2 to 1.6 g/kg/day, the position stands and trials behind it, why the per-meal split matters more than the daily total when appetite is low, and who should have a clinician set the number.

Training

Resistance training on a GLP-1: the minimum that works, how to start, and how to progress

You'll get: Guidelines ask for strength work on two or more days a week. The 2025 GLP-1 advisory asks for three.

What the physical activity guidelines and the 2025 joint advisory ask for, what the meta-analyses show about protein plus lifting, a two-to-three-day starting programme with progression rules, and the safety notes specific to therapy.

Index methodology

PubMed lists come from NCBI E-utilities and are not reviewed by us before they appear. Feeds are FormBlends Research data (CC BY 4.0); FormBlends sells compounded GLP-1 drugs and appears in the price index under the same rules as every other provider. Compounded drugs are not FDA approved.

Why this site exists

The pivotal GLP-1 trials came with diet and activity counselling that never makes it into the prescription: a 500 kcal/day deficit, 150 minutes a week of activity, regular visits. The DXA substudies showed that a share of the weight lost was lean tissue. The withdrawal trials showed what happens when the drug stops. Those three facts set the agenda here: protect lean mass, know what a normal curve looks like, and plan for maintenance before you need it.

How to read the numbers

Trial means are averages of hundreds of people on brand products at label doses. Half of those people did worse than the mean, and the range inside every arm was wide. Where this site applies a trial number to your weight it says so and shows the arithmetic. Compounded GLP-1 products are not FDA approved and are not interchangeable with brand products, and the trial numbers were not measured on them.
